Cannabidiol for Individuals at Risk for Alzheimer's Disease: A Randomized Placebo Controlled Trial
Research Narrative Use of cannabidiol (CBD) has been increasing rapidly across the United States, particularly among adults over the age of 55 with mild cognitive impairment and who might be at risk for Alzheimer’s disease (AD). The proposed randomized, placebo-controlled, double-blind trial will examine the effect of CBD biomarkers and behavioral measures associated with the progression of AD over 6 months. The information gained in this research is expected to inform public health about the effects of CBD in this population.

Effect of a potent and metabolically stable endocannabinoid receptor agonist on inflammasome-induced neuroinflammation in a comorbid mouse model of Alzheimer's disease and HIV
PROJECT NARRATIVE This administrative supplement application is in response to PA-18-591 and NIH notice, NOT-AG-20-034 about extending the active R01 projects to Alzheimer’s disease and related dementia (ADRD) research. With the increase in the life expectancy of people living with HIV (PLHIV), they face a burden of increased risk of developing Alzheimer’s disease (AD) due to the basal level secretion of toxic proteins such as HIV-Tat. Since there is no effective therapy either for AD or HIV-associated neurocognitive disorders (HAND), we want to test whether our recently discovered CB1 receptor ligand AMG315 with better potency and stability can mitigate neuroinflammation, amyloid, and tangle burden in a comorbidity model of AD and HIV.

Effects of HIV and Cannabis on Alzheimer's Disease-Associated Neuropathologies
NARRATIVE Little is known about the effects of HIV on Alzheimer's Disease (AD) in the aging HIV population. Likewise, whether cannabis exposure has a positive or negative effect on either condition is not known. Our proposed study will utilize a unique collection of postmortem brain tissues from our parent study to investigate AD-neuropathological changes in cannabis users and non-users who are HIV infected. Our study will lead to an enhanced understanding of relationships between long-term HIV infection and AD, and will stimulate additional research, including evaluation of the potential therapeutic use of cannabis for these diseases.

Pilot trial of dronabinol adjunctive treatment of agitation in Alzheimer's disease (AD)
PUBLIC HEALTH RELEVANCE: Patients with Alzheimer's disease (AD) often have severe agitation, and we have preliminary data suggesting that dronabinol (a medication derived from the active ingredient in marijuana and FDA-approved for severe lack of appetite) may also be safe and helpful in treating persons with AD and agitation. We propose to test the effect of three weeks dronabinol treatment in 80 patients at Johns Hopkins and McLean Hospitals, assessing primarily whether dronabinol helps agitation but also importantly assessing its safety.

Therapeutic Relevance of Cannabinoids for Alzheimer's Disease
Currently there are no effective drugs that prevent the death of nerve cells associated with neurodegenerative diseases such as Alzheimer's. This disease is very complex, and there are many factors that contribute to the patient's demise, yet most drug discovery projects use an approach that only addresses one of the many aspects of the disease pathology. In this proposal, a different approach to identify a drug candidate for AD is proposed, one that is based upon natural products that are known to interact with molecular systems in the brain that are compromised in AD.

Exploring the anti-inflammatory effects of cannabis on Alzheimer's disease associated neuropathology HIV infection and aging: An Autopsy Study
NARRATIVE With anti-retroviral therapy (ART), people living with HIV (PLWH) are living a normal life span and are aging, but the extended lifespan, poor ART penetration of the brain, neuroviral replication and chronic neuroinflammation may increase the risk for early onset of age-related neurodegenerative diseases like Alzheimer disease (AD) in PLWH. This proposed study will provide a detail examination of AD-associated neuropathology across age groups in PLWH to identify the underlying mechanisms of neurodegeneration, and whether cannabis use can mitigate early onset of AD-associated neuropathology in PLWH through its reported anti-inflammatory effects. Insights from this research could inform targeted interventions to prevent brain aging and neurodegeneration in PLWH in the United States and throughout the world.
